Initial Evaluation

Service Description

This comprehensive in-person evaluation is required for all new patients and takes place at our clinic. It is scheduled only after acceptance following completion of the online questionnaire. During this visit, your provider will review your health history, discuss your symptoms and goals, and create a personalized care plan tailored to your needs.
